Why Your Hormones Feel Like a Mess in Midlife: Oestrogen Dominance, the Progesterone Steal and the Inflammation Connection

Heavy periods that are not ideal at all. PMS that gets worse every year instead of better. Breast tenderness, fluid retention, broken sleep, a fuse that keeps getting shorter. And when you finally ask about it, you're told your bloods are normal and this is just what your age looks like.
I want to offer you a different explanation. Your age is only part of this story. The severity of what you're experiencing is being driven, to a very large degree, by inflammation - and that part is something we can actually work with.
Hormones Have Been in Every Symptom All Along
This week's podcast episode (coming on Wednesday) is the final one in my 8 Signs of Inflammation series, and I saved hormones for last deliberately. Because hormones have been quietly woven through every episode. Fatigue - oestrogen's role in how your cells make energy. Brain fog - oestrogen's neuroprotective function. Joint pain - its anti-inflammatory effects in your joints. Gut health - the estrobolome. Mood - progesterone's calming influence.
How you eat affects your hormones. How you sleep, move and manage stress affects your hormones. These were never separate conversations. They are one conversation, and this is where it all comes together.
Oestrogen Dominance: The Ratio Problem
Oestrogen dominance rarely means too much oestrogen in absolute terms. It means too much oestrogen relative to progesterone, and it's one of the most common hormonal patterns I see in women through their late thirties and early forties. It drives heavy periods, worsening PMS, breast tenderness, fluid retention and sleep disruption.
Here's the part almost nobody explains: inflammation feeds it through three separate mechanisms, all running at once.
Three Ways Inflammation Raises Your Oestrogen Burden
Note - this is before your menopause years.
1. More oestrogen gets made. An enzyme called aromatase converts androgens into oestrogen, and inflammation turns up its activity, particularly in fat tissue. More inflammation means more oestrogen production. The cycle feeds itself.
2. Less oestrogen gets cleared. Your liver processes used oestrogen so it can leave the body. When the liver is overburdened (alcohol, a high toxic load, processed food) its oestrogen-processing capacity drops. Used oestrogen recirculates instead of exiting.
3. More oestrogen gets reabsorbed. Once the liver has packaged used oestrogen and sent it to the gut, your gut bacteria decide whether it leaves. A disrupted microbiome produces beta-glucuronidase, which unwraps that packaged oestrogen so it's reabsorbed straight back into your bloodstream.
More produced, less cleared, more reabsorbed. That cumulative burden is what produces the symptoms you've been told are just your age.
The Progesterone Steal: Where Your Calm Went
Every one of your steroid hormones (oestrogen, progesterone, testosterone, cortisol) is made from the same raw material. When stress and inflammation are chronically high, your body prioritises cortisol production. Think of the cortisol factory taking the supplies meant for the progesterone factory. The result is chronically depleted progesterone - your calming, sleep-supporting hormone - and during perimenopause, this is happening at exactly the life stage when it's already declining.
The Thyroid and Insulin Connections
Low thyroid function impairs oestrogen clearance and changes how much hormone is available to your tissues, which is why I always ask for a full thyroid panel, never just TSH. And insulin resistance pushes androgen levels up, disrupting cycles even when standard bloods look "normal".
Why Two Women Have Completely Different Perimenopauses
The decline of oestrogen and progesterone through perimenopause is biological and normal. The severity is another matter. Two women with similar hormone levels can have completely different experiences. The difference, over and over again, is the inflammatory load the transition is landing on. Perimenopause amplifies whatever is already there.
Where to Start This Week
1. Cruciferous vegetables daily - broccoli, cauliflower, cabbage, kale. They support the liver pathways that clear used oestrogen.
2. One to two tablespoons of freshly ground flaxseed - the fibre binds oestrogen in the gut and escorts it out.
3. Look at alcohol with fresh eyes. Your liver cannot prioritise oestrogen clearance while it's processing alcohol.
4. Support progesterone production - magnesium is a critical tool, and reducing the stress load addresses the steal at its source.
5. Feed your gut diversity - 30 plants a week reduces oestrogen reabsorption.
Testing, HRT and Getting Proper Support
If you're considering HRT, come at it from an informed position. Looking at modern body-identical HRT, prescribed by a menopause-informed practitioner. What it cannot do is address the drivers - the inflammation, the clearance pathways, the progesterone steal, the thyroid, the insulin resistance. Those still need attention, HRT or not.
