It's Your Biology, Not Your Personality: The Inflammatory and Hormonal Drivers Behind Midlife Anxiety, Low Mood and Irritability

What I'm sharing here is not a replacement for that care. It's an additional lens. A piece of the picture that's so often missing from the conversation, and that, when it's finally understood, changes everything about how you relate to your own emotional experience.

Because here's what I want you to know from the start. Mood is not just in your mind. It's in your gut, your hormones, your immune system, and your inflammatory status. Let's look at what's actually driving this.

 

Your Mood Is Not Just in Your Mind

Mood changes driven by inflammation and hormonal disruption have a very specific character. There's the anxiety that turns up without a credible cause - the racing heart, the 3am waking with your mind already at full speed. There's the low mood that arrives when, by every objective measure, life is going well. There's the irritability, the snapping at the people you love most, followed by shame. And there's the emotional rawness, that sense of being on the edge of tears with no reserves left.

What all of these share is a nervous system that has lost its resilience. And in the vast majority of women I see with this picture, that lost resilience has a biological driver that has never been identified, let alone addressed.

 

The Inflammation-Serotonin Connection

For over two decades, research has been building around what's called the cytokine model of mood. In roughly one in four people experiencing depression, chronic inflammation appears to be a direct driver, not a downstream consequence of feeling unwell.

Here's the short version. Inflammatory cytokines - the chemical alarm signals your immune system produces - can signal the brain and switch on an enzyme called IDO. Think of IDO as a traffic controller that, when inflammation flips it on, redirects tryptophan (the raw material your body uses to make serotonin) away from serotonin production and down a different pathway. The result is less serotonin, more inflammatory compounds in the brain, and a mood that suffers. Not because of anything happening in your life. Because of an inflammatory immune response.

 

Your Gut Makes 90% of Your Serotonin

This is the one I most want to land. Ninety percent of your serotonin - your primary calming, mood-stabilising neurotransmitter - is produced in your gut. Not in your brain. In your gut.

That single fact changes the whole conversation. If most of your mood-regulating chemistry is made in your gut, then the diversity of your microbiome, the integrity of your gut lining, and the quality of the gut-brain communication are directly determining how you feel. When the gut is disrupted, serotonin production falls and mood follows. This is why so many women who heal their gut tell me their anxiety eased and they felt more like themselves, when all they came in for was the bloating.

 

Blood Sugar, Cortisol and the Anxiety You Feel in Your Body

Every time your blood sugar crashes - after a high-carb, low-protein meal that spikes then drops - cortisol surges to rescue the falling glucose. And every cortisol surge produces the physical signature of anxiety: heart rate up, alertness up, threat system on. If that happens several times a day, your nervous system is receiving repeated anxiety signals that have nothing to do with your life. They're happening in your bloodstream, and they feel identical to anxiety with a psychological cause.

Layer chronic inflammation on top and the HPA axis - your stress control system - loses its off switch. The stress response stays tonically on. Small things feel disproportionate. Recovery takes longer. That tension that never quite resolves starts to make sense.

 

Oestrogen, Progesterone and the Calm You Lost First

Oestrogen and progesterone are mood-regulating hormones, not just reproductive ones. Oestrogen supports serotonin and dopamine and actively calms neuroinflammation. As it wobles up and down through perimenopause, those supports become inconsistent, which is a big part of the emotional volatility so many women describe.

Progesterone is the brain's calming hormone. Inside the brain it becomes allopregnanolone, which enhances GABA - your brain's volume control. Here's the part that matters: progesterone often drops first in perimenopause, as ovulation becomes less consistent. That's why increasing anxiety, a mind that won't switch off, and deteriorating sleep are so often the very first signs of perimenopause, sometimes years before hot flushes or irregular periods. The calm goes first, and it's almost always misread as stress or a personality problem.

 

What Actually Helps (and Where to Start This Week)

The encouraging part is that all of this is workable. In this weeks podcast episode I walk through seven steps in detail, but if you do nothing else this week, start with two.

First, stabilise your blood sugar at every meal - protein, healthy fat and fibre together, never refined carbohydrates on their own. Pay special attention to a protein-rich breakfast, because it sets the biochemical tone for your whole day.. Second, consider magnesium glycinate before bed (typically 300-400mg), which supports GABA, calms neural overactivity and improves the depth of your sleep. These two alone begin shifting the biochemistry of anxiety within days.

From there: heal the gut for serotonin (remove refined sugar, aim for 30 different plant foods a week), support your vagus nerve with slow extended-exhale breathing, move your body consistently, and protect your sleep like the medicine it is.
